About Us

The Beneficiary Care Clinic is a primary care medical clinic that features an interdisciplinary approach to delivering high quality, comprehensive, evidence-based, compassionate care. The Beneficiary Care Clinic engages patients as partners in their health and wellness.

Our enrolled adult dependent patients are provided access to a team of health care providers – nurse practitioners, physician assistants, nurses, medics, and administrative staff – to develop a complete, personalized health care plan. This multidisciplinary approach aims to improve access, coordinate services, address questions/concerns, and ensure the care plan is delivered and effective.

Care Teams

The Beneficiary Care Clinic provides team-based primary care services. Your health care team includes:
  • Core Care Team – You the patient, and your assigned clinician. If your Core Care Team clinician is not available, you will be cared for by another qualified clinician in the clinic.
  • Extended Care Team – Front desk staff, registered nurses, other clinicians and nursing staff, and a clinical pharmacist.

  • Annual health maintenance
  • Longitudinal follow-up and management of chronic medical conditions
  • Appointments for new health concerns
